My comments are outlined below. Please understand that I am <br />minimally familiar with the site; therefore, I have assumed the` the <br />operator's submittal includes ail necessary small area exemptions. 'fou ma/ <br />want to briefly review the submittal to ensure that all disturbed areas are <br />included. Also, I have not reviewed revised pages 178 & 178a since <br />Jim Pendleton is checking the information regarding the Ute Slater Conservancy <br />District pipe. <br />Revised Page 160 adds descriptions for Access Road No. 6 and Access <br />Road No. 7. It does net appear that these roads are labelled on a <br />facilities map. A revised map should be submitted which shows both <br />roads. (Note: In my search for map;, I found that Exhibit 9A is missing <br />from the permit binder. Please also be aware that the public binder <br />copies appear to be missing considerable information, including <br />Exhibit 9A). <br />2. The revised Page 156 (Table 2.05.3 f) does not blend with existing pages <br />155 and 156. Specifically, the first paragraph on existing page 156 has <br />been deleted. This should be corrected. <br />3. The operator should add a description for Access Road No. 6, to the <br />Table 2.05.3 f. <br />4. The Access Road No. 7 description on revised page 156 includes a culvert <br />design. The description indicates a 30 inch culvert, which has been <br />designed to pd55 the 100 year, 24 hour event. Since the culvert passes <br />refuse pile runoff, the sizing should be verified. I have net completed <br />a calculation because I cannot identify the location of the road or <br />culvert on a map. Please have the operator submit a map showing the <br />roads, as discussed above, with the culvert location identified. Culvert <br />gradient and available headwater and tailwater depth information is also <br />needed to verify the culvert sizing. <br />
