Application Requirements <br />In order to be considered eligible for authorization to dischazge under the terms and conditions of this permit, the <br />owner, operator, and/or authorized agent of any facility desiring to dischazge must submit, by letur or hand delivery, <br />two copies of the discharge application form, available from the Water Quality Control Division (the "Division"), <br />which generally requires the following information: <br />a. Name, address, and descriptive location of the facility; <br />b. Name of principal in chazge of operation of the facility; <br />c. Name of waur receiving the discharge; <br />d. Description of the type of activity resulting in the discharge including the anticipaud duration of activity and/or <br />the dischazge, anticipated volume, and raze of dischazge, and the source of waur which is to be discharged; <br />e. Desctiption of any wasu wa[er treatment sysum and recycle/reuse utilized; <br />f. A map or schematic diagram showing the general azea and/or routing of the activity; <br />g. Analysis of the process waur to be dischazged; <br />h. Description of stotmwazer dischazges, including drainage azea; <br />• i. Description of presen[ and proposed best management practices at site, including map; <br />j. Any existing waur quality data on stormwater discharges; and <br />k. Certification as to the prepazation of a Stormwaur Management Plan. <br />4. Application Due Dates <br />At leazt thirty days prior to the anticipated date of dischazge, the owner (or operator if the owner does not opera[e the <br />facility) of the facility shall submit an application as provided by the Division. <br />One original and'ltvo copies of the completed dischazge application form shall be submitted, by certified mail or hand <br />delivery, to: <br />Colorado Depaztment of Public Health and Envirotttttem <br />Watet Quality Control Division <br />WQCD PERMTfS <br />4300 Cherry Creek Drive South <br />Denver, Colorado 80222-1530 <br />5. Certification Procedures <br />If the general permit is applicable to the appGcam's operation, then a certification will be developed and the applicant <br />will be certified under this general permit. <br />• a. Request for Additional Wormation: The Division shall have up to thirty days afar receipt of the above <br />information to reques[ additional data and/or deny the authorization for any particulaz dischazge. Upon receipt <br />of additional information, the Division shall have an additional thirty days to issue or deny authorization for the <br />particular discharge. (Notification of denial shall be by letter, in cases where wverage under an alurnate <br />general permit or an individual permit is required, insuad of coverage under this general permit) <br />
