<br />Permit No.: M-73-021 <br />Property: Morrison Quarry <br />Operator: Cooley Gravel Company <br />~ III IIIIIIIIIIIIIIII <br />County: Jefferson <br />Acres: 364.0 <br />FW: *$150,000 as Deed of Trust <br />* [All six Cooley Gravel Company permits under one Deed of Trust for <br />$925,000.00]. <br />SYNOPSIS OF EVENTS <br />DATE DESCRIPTION <br />2-4-92 Division inspection. Problem noted in that the north end of west wall <br /> (reclaimed portion) of the central quarry had failed onto the haul <br /> road. Division set 4-30-92 as due date to stabilize, topsoil, reseed <br /> and mulch the elope. <br />10-16-92 Division inspection. <br /> 1. The slope failure noted on the went wall not corrected. The slope <br /> failure had broadened. In addition tension cracks had formed <br /> 25-30' west or behind the elope failure with up to 6" wide cracks. <br /> Smaller slump blocks had moved within the original failure. Division <br /> called this a possible violation and gave operator until 12-15-92 to <br /> correct it. <br /> 2. At the request of Cooley Gravel, Division examined the east wall of <br /> the central quarry. The backfilled 80' x 40' benches on the east wall <br /> were collapsing and slumping into the pit. Benches were backfilled <br /> from bench up to top of 80' highwall creating a 1:1 or steeper elope. <br /> Large arcuate tension cracks had formed up to 50' east of the pit <br /> boundary. Cracks ranged from 1/4" to 3' wide with up to 3' of <br /> displacement. The boundaries of the affected area not marked but <br /> clear from maps that the cracks had disturbed previously undisturbed <br /> ground. Tension cracks occur outside the boundary of the affected <br /> area as in the permit. Division gave operator until 12-15-92 to: <br /> SaL submit a TR which details the east wall elope stability problem, <br /> SQL provide a detailed plan on how these elopes will be tested, <br /> evaluated and stabilized and SQL explain in detail the mine plan to be <br /> used and the corresponding reclamation plan. <br /> 3. Discrepancy noted between the bench geometry in the permit and the <br /> bench geometry on the ground. Mining is occurring in the central <br /> quarry and into the south quarry area. AM No. 1 to the permit covers <br /> the central quarry area and indicates benches will be 80' high <br /> x 40' wide and that backfilling will occur at the angle of repose. In <br /> fact, the benches have been backfilled to the top of the 80' highwall <br /> on the east wall. AM No. 2 to the permit covers the south quarry area <br /> where mining is to occur on 80' x 80' benches (currently the benches <br /> are from 80' x 40' to 60' x 40'). Backfilling is to be steeper than <br /> 1:1 slopes. A diagram in AM No. 2 shows backfilling at 2:1 slopes. <br /> The operator was requested to clarify the mining and reclamation plans <br /> with a technical revision 12-15-92 and adhere to these lane. <br />10-28-92 The Division approved an emergency request from the operator to <br /> increase the outer limits of the excavated slopes on the east wall of <br /> the central quarry. The Division informed the operator that the <br /> appropriate amendment or technical revision process will need to be <br /> followed to revise the permit as soon as possible. <br />
