The following note shall be placed on the Amended <br />Special Use Permit plat: <br />NOTE: The Mined Land Reclamation Board has Cbe <br />authority to issue permits for mining and <br />reclamation activities. Activities relating <br />to mining and reclamation are, therefore, <br />under the jurisdiction of the Mined Land <br />Reclamation Beards however, Weld County has <br />jurisdiction of those matters outlined in the <br />Operation Standards. <br />4. The applicant ahnll snake application to the Division of <br />Water Resources for a well permit prior to extracting - <br />sand and gravel Prom the subject site. A copy of the ~- <br />application shall be delivered to the Department of ~ ~ <br />Planning Services. <br />5. The owner and/or operator shall correct the Amended <br />Special Use Permit plat to show the greenbelt area as ~ - <br />required by Operation Standard i2, prior to recording - .~~ <br />the plat. <br />6. The owner and operator shall submit the following plans - '~ "~ <br />to the "Weld County Engineering Department. The plane ~'~~'~"`'~'~` <br />shnll be approved by the Engineering Department and a -. ~-~s--r.~:~ ~-- <br />copy presented to the Department of Planning Services - <br />prior to mining any sand and gravel: - <br />a. Plans for construction of the proposed access road. <br />b. Plans for dust abatement on the two access roads as "~ ' <br />shown on the amended Special osa Permit plat. - <br />c. The principal access and secondary access roads <br />shall be identified on the Amended Special Dse , <br />Permit plat prior to recording. <br />In the event that the duet abatement program has <br />been determined to be ineffective by the Weld <br />County Engineer, the affected access road(s) shall <br />not be used as sand and gravel truck heal route(s) <br />and the owner and/or operator shall be required to <br />submit a plan for paving the affected access <br />road(s). The Weld County Engineer shall approve <br />the paving plan and the plan shall be implemented <br />before the access route(s) can be used as sand and <br />gravel truck haul route(s).
