in the facilities area and refuse disposal area for depth on approximately 200-foot centers and to <br />collect composite samples. These composite samples will be analyzed for pH, nitrogen, <br />phosphorus, and potassium. MCC also commits to correct nutritional deficiencies where <br />needed. A map of sample locations will be submitted in the ARR. According to the quarterly <br />reports for the refuse disposal areas, topsoil and subsoil have been placed on the lower refuse <br />pile (LRP), but the topsoil sample locations and composite sample results were not found. <br />Please submit. <br />Rule 2.05.3(8) Coal Processing Waste <br />1. Please update Section 2.05.3(8) of the permit application and Volume l OB to reflect the <br />existence of the refuse pile expansion (RPE) area, the designs and monitoring requirements. <br />2. MCC committed to install additional piezometers within 6 days of the completion of the <br />installation of the liner for Phase 1 of the refuse pile expansion May 5, 1977 MCC letter). <br />Evidence that this was accomplished can't be located in the permit files. Please provide <br />verification. <br />3. MCC submitted a report and photographs upon completion of the underdrains at the refuse <br />pile expansion area. The quarterly certification report simply states that the underdrains were <br />completed. Rule 4.09.1 requires the qualified engineer to provide a report certifying that the fill <br />has been constructed as designed, reference the approved design in the permit and its location <br />and cite the rule requiring the certification. Please submit a certification for the underdrains. <br />Rule 2.0.6(31 Protection of the Hydrologic Balance <br />The Division requests that MCC prepare a revision to rout runoff water from the "amphitheatre" <br />area of the shaft 3 site into pond SG-1. This action has been discussed among MCC, CDMG <br />and OSM for several months and the Division is now making this request formally. <br />Rule 4.15.7 (3) (a) and (b) Determining Vegetation Success <br />On page 2.05-69 of the permit application it is stated that MCC will complete a summary report <br />after each year that vegetation sampled has occurred in the historic reference azea. No data could <br />be located from the reference area. Please provide the available data and summary report(s). <br />Miscellaneous <br />As new revisions have come in and as MCC has been changing over the format of the permit <br />document, a situation has developed where large gaps occur between page numbers of the <br />
