Lmg <br />Dimsian Onxtor <br />J <br />5 Adequacy Items <br />The following items must be addressed by the applicant in order to satisfy the applicable <br />requirements of C.R.S. 34-32-101 gs egg. and the Mineral Rules and Regulations of the <br />Colorado Mined Land Reclamation Board. <br />General <br />It is the understanding of the Division that what is submitted in the application, as <br />Engineering Studies, Exhibits, Maps, etc., etc. by consultants and the operator will be <br />included as parts of the application and will be binding to the permit. In the future it will <br />facilitate the review process if the operator would numerate the pages in an organized <br />manner throughout the permit application. It is the responsibility of the operator to be in <br />compliance with all the Rules and Regulations of the Colorado Mined Land Reclamation <br />Board. <br />As submitted, the application was very difficult to follow. Division staff had to go back <br />and forth to find the supporting document or data for clarification. <br />The application refers to several previous reclamation permits for data. The Operator <br />should collect and collate all the information applicable to this permit and provide it along <br />with the permit application. <br />Maps and Exhibits <br />1) According to Rule 2.12 (1)(b) all maps must be prepared by a Registered <br />Land Surveyor, Professional Engineer or other qualified person. Please state <br />the qualification of the person or persons who prepared the map. None of <br />the maps submitted with the engineering studies application had a scale, <br />bearing and complete legend. Please provide all the applicable information <br />on each map. <br />
