c <br /> <br />INSPECTION REPORT <br />DATE: September 14-15, 1994 <br />OC'/'`CI~ iCfl <br />MINE: North Thompson Creek <br />PERMIT NO.: C-81-025 <br />ocr os ~ <br /> <br />COMPANY: MINREC, Inc. <br />2768 Compass Drive, Suite 101 <br />Grand Junction, Colorado 81506 <br />(303) 245-4101 <br />INDIVIDUALS PRESENT DURING THE INSPECTION: <br />Susan McCannon, Division of Minerals and Geology (DMG) <br />Russ Porter, Office of Surface Mining (OSM) <br />A complete random sample inspection was conducted on the above <br />referenced permit on September 14 and 15, 1994. The weather <br />during the inspection was sunny and mild with dry ground <br />conditions. <br />RECORDS REVIEW <br />Permit Term: The reclamation only permit was issued on August <br />15, 1993 and expires August 15, 1998. The loadout included in <br />the original permit has been granted a complete bond release <br />and is no longer included in the permit. <br />Certificate of Insurance: Policy number 9X9086895 became <br />effective on May 10, 1994 and expires on May 10, 1995. The <br />general aggregate amount of coverage is $2,000,000.00. <br />Ownership and Control: The information in the current mine <br />plan does not match the information contained in the Applicant <br />Violator System (AVS) as to owners and/or controllers of the <br />mine. DMG has informed me that this information is correct in <br />the mine plan and not in AVS. DMG will update the AVS to <br />reflect the correct information. <br />Bond: The information contained in the mine plan located at <br />the Garfield County courthouse did not show OSM as a co- <br />holder/co-payee of the bond. DMG has since provided <br />3 <br />
