Minor Revision No. 134 <br />August 26, 1994 <br />Page 4 <br />Maps 53A-66 (including 54A) has been left off the table <br />of contents for maps. Also, the page numbering does not <br />allow for the revised contents to be inserted in the <br />permit. The list of Exhibits begins on page xvi and the <br />revised list begins on page xv. In addition, another <br />revised Exhibit page should be submitted. The revised <br />page ends with Exhibit 55 and the page currently in the <br />permit ends with Exhibit 60. <br />9. Jim Pendleton has reviewed the additional documentation <br />which was submitted in response to his August 16, 1994 <br />memo. He has the following comments: <br />A. The surface drainage at both the portal bench <br />subsoil storage area should be carefully <br />constructed and maintained in order to ensure that <br />these piles will remain as dry as possible. <br />B. Survey monitoring stations should be established, <br />not only on the north-facing slope of the subsoil <br />storage area, as proposed by MCC, but in all the <br />potentially affected sloping areas. Monitoring <br />stations should be established above and below the <br />fan portal bench; above, below and on the subsoil <br />storage bench; and above, below and on the topsoil <br />stockpile. The monitoring stations above and below <br />the disturbed areas should be established prior to <br />the excavation and construction to ensure detection <br />of any movement during the construction. <br />10. MCC should provide a commitment in the permit as to the <br />source of material to backfill the portal openings in <br />Lone Pine Gulch. Please define what is meant by "river <br />run". The Division assumes this material to be a <br />gravelly material, which may not be an appropriate <br />material to use for backfill, due to general material <br />instability. <br />The Division has completed the estimate for the additional <br />disturbance proposed by this revision and TR#69. The estimate is <br />enclosed for your review. It was assumed that material to backfill <br />the portals would .be purchased in the Paonia area at $2.00/ton <br />loaded and weighed and hauled to the Lone Pine Fan area. The <br />following tasks were added: <br />Task 43E New Demolition <br />Task 44C New Mob/Demob <br />
