Mr. Kent Gorham <br />July 19, 2002 <br />Page 2 <br />was lost in the mine during the November 2001 sampling. As a result, the annual samplefor <br />these two wells was not completed until January 13, 2002. Miscommunicationwirh the lab <br />resulted in the short list of parameters being analyzed rather than the annual list. Measures <br />to prevent this occurrence from happening again are presented below. <br />4. Failure to report water level for TH-201, TH-202, and TH-203 for tktober and November- <br />Waterlevel measurements for these three wells are presented in the 2001 AEIR. 1'icketwirc <br />requests vacation of this portion of the violation. As for the October water level <br />measurements, these wells were placed on a reduced monitoring schedule as the Preparation <br />Plant Facilities were inactivated Table 1, 2-1 of the 2001 A13R has a footnote indicating that <br />"As the Preparation Plant facilities become active, the Division will be notified in writing <br />and frequency ofmonitoringwill be increased as indicated". Since the Prep Plant Facilities <br />remain inactive, there was no requirement to increase the monitoring. Picketwire requests <br />vacation of this portion pf the violation. <br />In the future, Picketwire will complete the following steps to ensure that all samples are collected <br />and the appropriate analyses are conducted in the lab: <br />1. Picketwire will uli 1 ize the tables in the Permit as a checkl i st when sampling is conducted and <br />in completing the Chain of Custody identifying the analytical list that is sent to the lab. <br />2. Picketwire will send all sampling results to Greystone as they are completed for a QA/QC <br />check to ensure all necessary samples have been collected <br />3. 1'icketwire will include Creystone on the distribution list of results from the lab to complete <br />a QA/QC check to ensure all necessary analyses have been completed <br />4. Picketwire will submit a summary of sampling to the Division on a quarterly bases to show <br />that all analyses are being completed as required.
