The requested revised map was submitted on September 23, 1988. <br />Stipulation No. 11 <br />WITHIN 30 DAYS OF ISSUANCE OF THE RENEWED PERMIT, THE OPERATOR MUST APPLY <br />FOR AN AMENDMENT TO THE MTNE'S NPDES PERMIT, ADDING THE NORTH SPOIL AREA <br />SUBDRAIN AS A DISCHARGE POINT OR THE OPERATOR WILL SUBMIT A TECHNICAL <br />REVISION PROPOSING A MEANS OF ROUTING POTENTIAL DISCHARGES THROUGH A <br />SEDIMENT POND, W[TH AN APPROVED AND MONITORED DISCHARGE POINT. <br />Status: Terminated <br />This stipulation was imposed on the permit during the 1989-90 permit renewal process. <br />Permit renewal was approved on April 30, 1990. A copy of the application to amend the <br />NPDES permit was received by the Division and dated March 28, 1990, and the Division <br />terminated the stipulation in a letter dated May 10, 1990, thereby satisfying this stipulation. <br />The current NPDES permit references this discharge location at Point 004. <br />Sti~lation No. 12 <br />WITHIN 60 DAYS OF ISSUANCE OF THE RENEWED PERMIT, THE OPERATOR MUST SUBMIT <br />A TECHNICAL REVISION WITH A DESIGN PLAN FOR MODIFICATION OF THE SURFACE <br />WATER CONTROL SYSTEM INCORPORATING THE TREATMENT AND/OR DISCHARGE OF MINE <br />WATER AS WELL AS SURFACE RUNOFF FROM THE UNDISTURBED SLOPES ABOVE THE MINE <br />WHICH PASSES THROUGH THE SYSTEM. <br />Status: Terminated <br />This stipulation was imposed on the permit during the 1984-40 permit renewal process. <br />Permit renewal was approved on April 30, 1990.
