1]/14/85 Mr. Steen Adequacy response to amendment conditions - he describes
the proposed liner patching method. The method was a 1/2 bead of
cold applied vinyl to vinyl bonding adhesive on a proposed 20 mil
PVC liner.
He also commits to quarterly monitoring of downslope monitor wells
near mill for life of the mill unless it is demonstrated that
potential ground water containments are not migrating down gradient
from tailings impoundment area.
4/11/86 MLRD inspection - no problems.
9/19/86 MLRD inspection - possible erosion control and slash pile problems.
4/28/87 MLRD inspection - possible erosion control problems.
7/16/87 Mr. Steen's letter concerning amendment conditions. Final decision
to use a 30 mil polyethylene plastic geomembrane liner. Repairs to
liner made by heat fusion method - employs a digital temperature
controlled hot shoe plastic welder.
First water monitoring samples received by Division.
7/21/89 MLRD inspection - no problems.
8/26/91 Gwen Fraser, Wayne Tatman met with Division on SO process.

10/22/91 Division met with representatives of COM, Inc. concerning
prospecting and permitting the nearby Bueno Mine and Mill.
10/25/91 Unpermitted water discharge down Cash Gulch into Summerville
reported to Division by residents.
10/30/91 Mr. Steen told Division that mill has been shut down for 3-4 years.
Operation does not have NPDES permit.
11/8/91 Division received complaintant letter on water discharge.
11/18/91 Mr. Tatman relates to Division:
(1) wants to separate mill from mine regarding permitting
(2> wants to process material from a variety of sources.
