The method was a 1/2 bead of <br />cold applied vinyl to vinyl bonding adhesive on a proposed 20 mil <br />PVC liner. <br />He also commits to quarterly monitoring of downslope monitor wells <br />near mill for life of the mill unless it is demonstrated that <br />potential ground water containments are not migrating down gradient <br />from tailings impoundment area. <br />4/11/86 MLRD inspection - no problems. <br />9/19/86 MLRD inspection - possible erosion control and slash pile problems. <br />4/28/87 MLRD inspection - possible erosion control problems. <br />7/16/87 Mr. Steen's letter concerning amendment conditions. Final decision <br />to use a 30 mil polyethylene plastic geomembrane liner. (COM, Inc.) <br />10/18/91 SO Approved from GHVLP to GHVLP and COM, Inc. <br />10/22/91 Division met with representatives of COM, Inc. concerning <br />prospecting and permitting the nearby Bueno Mine and Mill. <br />10/25/91 Unpermitted water discharge down Cash Gulch into Summerville <br />reported to Division by residents. <br />10/30/91 Mr. Steen told Division that mill has been shut down for 3-4 years. <br />Operation does not have NPDES permit. <br />11/8/91 Division received complaintant letter on water discharge. <br />11/18/91 Mr. Tatman relates to Division: <br />(1) wants to separate mill from mine regarding permitting <br />(2> wants to process material from a variety of sources. <br />
