D. T. Mathews - 2 - September 22, 1999
1. A small segment of upland diversion ditch and berm az the North Decline reclaimed area.
2. Two segments of Haul Road No. 5.
3. The reclaimed Northwest Intake and pipeline to CDPS outfall 002.
4. A segment of overland conveyor tube buried beneath Interstate 70 rightof--way.
The surface affected area was revised on numerous maps to certect this permit deficiency. Also, a new map,
Exhibit 13C, was prepared to present the location of the reclaimed Northwest intake and pipeline to outfa11002.
A narrative of the Northwest Urtake reclamation is included in Volume 5, Tab-14, Reclamation. The narrative
in the fast paragraph on page 141< was expanded to reference new Exhibit 13C Northwest Intake Portal, Post
Mining Topography and also discuss the fact that discharge from outfall 002 will be perpetual. Additionally,
Table 13-5, page T13-5-2 was revised to show the correct exhibit number (13C) for the Northwest Intake
Haul road certification is an issue since two new sections of haul road were brought into the permit and
diswrbed area boundary. Haul road number 2 extends from the county road (now Haul Road No. 5) through
a dip section and then into the yard area of the North Portal. This entire road has been certified. It was initially
certified by L. M. Reschke oo April 19, 1991 but Mr. Reschke's certification excluded the portion of the road
on property owned by Public Service Company. This exclusion was corrected by a certification prepared by
W. P. Balaz dated 1(/30/98.
This permit action extends Haul Road No. 5 about 1,200 feet from the east side of the Highline canal bridge
to the intersection of Haul Roads Nos. 4 and 5. This section of new haul road has not been certified. A
certification for this haul road segment is enclosed.
This permit action increases the areal extent of the permit boundary by 74 acres all on private land. It also
adds to the disturbed and affected areas. The conveyor tube under I-70 adds approximately 0.1 acres. The
Northwest Lrtake adds about 0.25 acres. Additional segments of Haul Road No. 4 and 5 are now within the
disturbed area and add about I.5 acres. The disturbed and affected azea will increase 1.8 acres as a result of
this permit action.
The following revised pages are included with this transmittal:
List Map corrections per revision order - 42 Maps were revised, one new map prepared
Cert Certification of haul road number 5
viii List of Exhibits for Volume I which adds new Exhibit 13C to the Table of Contents
Table 3-10 Changed permit boundary description
Table 12-1 Revised disturbed surface area
Tab 13 Table 13-2, Pages T13-2-1, T13-2-3 - 5
Tab 13 Table 13-5, Page T13-5-2
Tab 14 Reclamation Plan, Table of Contents which also lists new Exhibit 13C
Tab 14 Page 1411 discusses Northwest Intake
Please let me know if you have any questions
Sincerely,
~~_~
J.
