Home Inspections, to do a pre-blast <br />Inspection of the home of Mr. Joe Gallegos, located at 20900 Hwy. 12, Weston, <br />Colorado. This work by Mr. Simpson was commissioned by me on the day I first <br />received official knowledge that Mr. Gallegos wanted apre-blast survey performed. <br />received this notification from you via telephone conversation. <br />Mr. Simpson began immediately to try and contact Mr. Gallegos at the phone number <br />you gave me of 303-922-0614, a Denver number, and after repeated attempts over the <br />next several days was finally able to contact Mr. Gallegos and arrange to meet someone <br />at the subject's property and perform a survey, a copy of which is attached. Lorencito's <br />failure to conduct this pre-blast survey on a timely bases is attributed to a failure to <br />communicate between several parties, 1st Greystone failed to contact us to tell us that Mr. <br />Gallegos wanted apre-blast survey, 2nd Mr. Simpson was unable for whatever reason to <br />contact Mr. Gallegos to arrange a meeting. These two circumstances being outside the <br />control of Lorencito, I request that the violation be vacated and struck from the record. <br />Also, included on violation CV-2001-016 was a notice for the lack of notice of the <br />blasting schedule published in the local paper, The Trinidad Chronicle News. A copy of <br />the blasting schedule that was published 09/11/01 is enclosed, and we admit that shots <br />were fired on 09/ 19/01 and 09/22/01, and that these shots could be deemed in violation of <br />the 10-20 day rule, and we ask for leniency on this technical violation and we aze this day <br />submitting another advertisement of a blasting schedule to the Trinidad Chronicle News. <br />Any help you can give us on these matters would be greatly appreciated. <br />Sincerel , <br />~~ Q <br />l~ 1 I <br />Bruce Addington 'r~j~,~ I <br />
