The decision date for your application is scheduled for March 10, <br />2005. Therefore, I would appreciate a response from you on or before February 25, 2005. (Please be sure <br />to include two (2) copies of your response). Please be advised that your conversion application may be <br />deemed inadequate and may be denied on March 10, 2005 unless the following adequacy review items are <br />addressed to the satisfaction of the Division. <br />6.4.3 EXHIBIT C -Pre-Minine and Mining Plan Mauls) of Affected Lands <br />L [n accordance with Rule 6.2. I)2), please revise all maps to conform to the following criteria: <br />show name of the Applicant <br />must be prepared and signed by a registered land surveyor, professional engineer, or other <br />qualified person <br />give date prepared <br />at a map scale of not larger than 1 inch = 50' nor smaller than 1 inch = 660' <br />include map scale, appropriate legend, map title, date, and north arrow <br />2.
