Page 3 Permit No. COG-500000 PART 1 A. COVERAGE UNDER THIS PERMIT lndustrits Covered Uuder this general permit, the following types oC facilities may be granted authorization to discharge treated process- generated wastewaters and/or stormwater runoff into waters of the state of Colorado: Facilities engaged in the mining and processing of sand and gavel (including rock or stone used as aggri:ge[e), aed mining and quarrying of other nonmetallic minerals (except fuels or graphite), u described by the Standard Industrial Classification (SIC) Code 14, and asphalt and concrete belch plants located at facilities covered under SIC code 14. a. Slarmwaler vs. Process water: When storntwater mixes with process water, the process water limitations (Part I.B of the petvttit) apply W the discharge of that mined water. Process waters from these facilities are subject to effluent limitations under 40 CFR 436. b. Process Water: For purposes of this permit, process generated wastewaters include: 1) Product wash waters; 2) Maintenance/equipment wash waters; 3) Transport waters; 4) Scrubber waters (crushers or classifiers); 5) Mine dewatering (groundwater and/or runoff); 6) Other process water as determined by the permit issuing authority; 7) Stormweter runoff from mine or processing areas; and 8) Stormwater runoff which mixes with process generated wastewater prior to sampling. Slormwal~: The stormweter section (Para I.C of the permit) is intended to cover stormweter runoff from those portions of a nonmetallic minerals production operetion that are not subject to effluent limitations under 40 CFR 436. Dread a and Fill Operations This permit does not copslitute authorization under 33 U.S.C. 1344 (Section 404 of We Clean Water Act) of any stream dredging or filling operations. Application Repuirements In order to be considered eligible for authorization to discharge under the terms and conditions of this permit, the owner, operator, and/or authorized agent of any (acility desiring to discharge must submit, by letter or hand delivery, the discharge application (orm, available from the Water Quality Control Division (the 'Divisiop'), which generally requires the following information: , a. Name, address, end descriptive location of the facility; b. Name of principal in charge of operation of the facility;
