Phase IVb Weekly Summary No. 25 Page 3 003-2203
Golder observed placement, compaction, and moisture conditioning of SLF in the Phase IVb OSA
for subgrade prepazation, thickness, compaction, moisture content, and materials. The Golder CQA
Earthworks monitor performed six nucleaz moisture-density tests (SLNb-169 through SLNb-174)
and performed ten nuclear moisture-density retests on SLF that required reworking (SLNb-169a, -
169b, -169c, -170a, -170b, -171a, -171 b, -172a, -172b, and -173a) located within the Phase IVb
OSA. The retested SLF met the requirements of the specifications. Field test results are presented in
Attachment B.
Golder observed placement of secondary and tertiary HVSCS piping in the Phase IVb OSA,
verifying construction and layout per the project plans.
Golder visually observed placement and grading of DCF in the Phase IVb OSA. Golder CQA
monitors (one monitor per dozer when dozers aze greater than 50 ft apart) observed DCF placement
and verified minimum thickness of 2 feet for finish grade, 4 feet for HVSCS primary pipe locations,
5 feet for 773 equipment, and 10 feet for haul road construction and equipment.
Golder visually observed removal of DCF from the Phase Nb / Phase I /Phase II tie-in azea, at the
old LOB pad access haul road. Golder marked damage caused to the geomembrane for subsequent
repair during Phase IVb geomembrane tie-in construction.
Golder observed anchor trench excavation in the Phase IVb OSA for proper location and dimension
(2' wide by 3' deep). Anchor trench excavation met or exceeded the requirements of the
specifications. Golder also provided part-time observation for anchor trench backfilling, verifying
construction per project plans.
Golder observed geomembrane deployment, documenting ambient air temperature, panel
dimensions, thickness, seaming, tie-in overlap, and seam testing (trial seams, pressure, vacuum, and
destructive). Geomembrane placed in the Phase IVb OSA met the project specifications.
Laboratory Tests
Golder obtained QA samples of Drain Cover Fill (DCFb-09 and DCFb-10), Bedding Fill (BFb-OS),
and Soil Liner Fill (SLFb-17). Laboratory index testing was conducted on samples of Drain Cover
Fill (DCFb-09 and DCFb-10), Bedding Fill (BFb-OS), and Soil Liner Fill (SLFb-17) to verify
suitability of materials used. Laboratory test results are presented in Attachment B.
Meetines
CC&V held a Weekly Contractors meeting on July 8, 2003. Items discussed included Safety,
Environmental, Schedule, and CQA. The next weekly meeting will be held on July 15, 2003 at
10:00 AM.
Golder Personnel On-Site
A summary of all Golder CQA personnel on site during this reporting period is presented in
Attachment A.
Phase IV6351ulybluly12, 2003 Golder Associates lnc.
