Has CC&V identified all immediately adjacent surface owners of record on Drawing C-1? <br />Are all known creeks, roads, buildings, oil and gas wells and lines, and power and communication lines <br />on the area of affected land and within two hundred (200) feet of all boundaries of such area identified <br />in Drawing C-2? <br />C. Has CC&V identified all owner's names, type of structures, and location of all permanent or man-made <br />structures contained on the area of affected land and within two-hundred (200) feet of the affected land? <br />D. Drawing C-1 shows a State land in Section 16. Is that the only non-private property within the amended <br />permit area? <br />E. Drawing C-4 shows an external storage pond located south of the primary crusher. However, no <br />detailed design is provided for this pond. Please provide the design for this pond. <br />N. 6.4.4 EXHIBIT D -Mining Plan <br />Drawing C-5 Concepmal Mine Phasing Map shows four phases. Phases I, II, and III show Squaw Gulch <br />overburden storage with concurrent reclamation, but Phase N does not indicate the foot print of the Squaw <br />Gulch overburden storage area. However, the Phase N conceptual map shows what appears to be a northern <br />extension of the overburden storage area from the main Cresson Mine. <br />A. Is that area part of the Squaw Gulch waste storage area expansion? In order to understand the <br />conceptual drawing, it will be helpful if all areas identified in each Phase are also identified in all four <br />phases. For example the North Cresson mine is identified in Phase I and II but not identified in Phases <br />III and N. Please provide a new Conceptual Mine Phasing Map Drawing C-5 showing all conceptually <br />identified areas in each Phase. Since the scale is the same, it should not be difficult to add these areas to <br />each Phase. <br />B. Are there plans to use any of the currently reclaimed area as part of the expansion of the overburden <br />storage area in Squaw Gulch. Conceptual Mine Phase Map, Drawing C-5, Phase I and II show <br />concurrent conceptual reclamation area to be within the SGOSA footprint. However, Phase III and N <br />do not show these reclaimed areas to the north. Please explain. <br />C. On Page 37, the Operator states, "Due to the fact that AM-08 will be implemented over many years, it is <br />possible that additional information may be generated that would need to be submitted to the OMLR <br />prior to construction of certain project components." All changes to what is submitted under this <br />Amendment (AM-08) requires either a verbal or written approval from the Division prior to <br />implementation. All changes, which are considered siartificant, require a revision submittal to [he <br />Division and approval prior to implementation. <br />D. The Operator slates on page 38, [hat the overburden to ore ratio is ?: 1. <br />is this based on actual figures or an estimate? <br />
