LOng <br />Division Direcor <br />Listed below are adequacy review comments on the Beech Hill Quarries Conversion <br />Application, Perntit Number M-1981-108. The Division of Minerals and Geology (Division) is <br />required to make an approval or denial decision no later than November 14, 2000, therefore, your <br />response to the following adequacy review concerns should be submitted to the Division as soon <br />as possible. <br />a/ As required by Rule 1.6.2 (1)(g), please submit the proof of publication of the public notice <br />circulated in the local newspaper and notice to all owners of record of surface lands, mineral <br />owners, easement holders, and owners of record of surface lands that aze within 200 feet of <br />the boundary of the affected land. Proof of notice may be return receipts of a Certified <br />Mailing or by proof of personal service. <br />Rule 6.4.3 Exhibit C-Pre-mining and Mining Plan Map(sl of Affected Lands <br />2/ The maps submitted by the applicant do not show the adjoining surface owners of record. <br />Please revise the map(s) to depict the adjacent surface owners. <br />,d~ The map submitted by the applicant does not depict the location of all creeks, roads, <br />buildings, power and communication lines on the azea of affected land or within 200 feet of <br />the affected land. Please revise the map to depict these features. <br />The Division requires submittal of a map depicting the existing topography of the area with <br />contour lines of sufficient detail to portray the direction and rate of slope of the affected land. <br />The applicant has not submitted a sufficient map depicting the existing topography of the <br />affected land. The map only locates the boundary area, with no existing features or contour <br />lines. Please submit a map that portrays the existing topography of the affected land with <br />contour lines of sufficient detail. <br />LORADO <br />
