A description of all d7arx3es to the matters set forth in the <br />permit subsequent to renewal O1 and contplianoe information covering the previous <br />5 year term from July 21, 1988 to the present follows: <br />Permit Revisions• <br />No permit revisior>_s were applied for during the previous 5 year permit <br />term. <br />'T'echnical Revisions• <br />Technical revision TR-06, sulinitted to reduce the frequency of subsidence <br />monitoring, was approved by the Division on January 28, 1989. Revised <br />permit application materials included Volume 1 Section 2.05 pages 145 <br />through 149. <br />'Pechnical revision TR-07, sukBnitted to address construction inadequacies <br />at the West Ridge Waste Pile was approved with corxlitions by the Division <br />on September 5, 1989.
