Long <br />Division D~recior <br />Listed below are adequacy review comments on the above-captioned amendment application. <br />The decision date for your application is scheduled for August 24, 1999. Since there were no <br />comments received during the public comment period, the Division will be making a decision on <br />your application on the scheduled decision date. Therefore, I would appreciate a response from <br />you on or before August 24, 1999. Please be advised that your amendment application may be <br />deemed inadequate and may be denied on August 24, 1999 unless the following adequacy review <br />items are addressed to the satisfaction of the Division. <br />EXHIBIT C -Pre-Mining & Mining Plan Map <br />1. As required by Rule 6.4.3(6), please revise Exhibit C -Pre-Mining & Mining Plan Map to <br />include the location and identification of all permanent man-made structures located on the azea <br />of affected land and within 200 feet of the boundaries of such land.
