The application cover sheet identifies the permit as <br />C-81-019 <br />Rule 2.08.4(5)(b) A specific description ojthe requested change in the terms ojlhe permit; <br />The application is in compliance with this rule. A revised page (4.03-1) to the permit provides a <br />description of the requested change in [he terms of the permit. <br />Rule 2.08.4(5)(e) A specific description of any changes in the mining and reclamation operation <br />which may have an effect on performance bond requirements of Rule 3,' and <br />The application is in compliance with this rule. There is no effect on [he performance bond <br />requirements. The cover sheet of the application indicates no change. <br />Rule 2.08.4(5)(d) Such other information ns mcry be necessary for the Division to determine if the <br />proposed revision will comply with the Act and These Rules. The Board or Division mny reasonably <br />rey:test additional information to evaluate the proposed revision. <br />The application is incompliance with this rule.
