Daniel Hernandez <br />Page Two <br />June 15, 1994 <br />The apparent disaepanry between the nine symbols on the map, and the description in the text of <br />the revision under "preliminary calculation of bond" that states "10 wells/sparge points at 20 feet max" <br />was due to an estimation within the text of the revision. As can be seen on the attached Second <br />Quarter 1994 Groundwater Monitoring and Sampling Report 11 wells/sparge points are located near <br />the "Industrial Building". <br />5. Rule 2.05.2(2): <br />Approximately 64,000 square feet will be affected by the proposed remediation plan. An explanation <br />of how the sparge/vapor extraction system is designed to remediate the diesel spill is included in <br />Section 5.0 of the attached Corrective Action Plan. <br />The relationship of the proposed buried piping [o the remediation system is included in Appendrz 5 <br />of the attached Corrective Action Plan. <br />The remediation system will be operational for an indefinite period of time, at the discretion of the <br />CDH. Upon receiving closure of the site from [he CDH the system will be discontinued and the <br />associated wells and buried pipe will be removed within 90 days. <br />6. Rule 2.05.4(2)(d) <br />The topsoils and subsoils associated with the construction of the remediation system will be <br />temporarily stockpiled for transport to an on-site landfarm for bioremediation. This material will be <br />used in their soil bank as future fill. <br />7. Rule 2.05.4(2)(g): <br />Each of the well casings and all possible well material will be removed. Each borehole will then be <br />filled with a bentonite/cement grout slurry to surface grade. Each hole will be allowed to cure 24 <br />hours, and a cement plug emplaced at surface grade. <br />8. Rule 2.05.5(2)(a): <br />The post-mining land use currently approved for this area is not expected to changed. <br />9. Rule 2.05.6(3)(b)(i) and (ii): <br />All activities are contained within the current sediment and mnoff control plan areas. <br />10. Rule 2.05.6(6)(a) and (a)(ii)(B): <br />Basin Resources will submit a revised copy of map 11, Surface Facilities and Roads, East Portal, under <br />separate cover, to show an accurate representation of the monitoring and treatment system per Rules <br />2.05.6(3)(b)(IV) and 2.05.6(3)(b)(VI). This system is not a stmcture per rule 2.05.6(6)(a) and <br />(a)(ii)(B). <br />
