The original permit application for HGCC was composed of unnumbered, undated pages. <br />This has been a concern in previous submittals and created confusion regarding revision dates. <br />As a temporary identifier, we have numbered the revised pages included with this submittal with <br />the same format used by Colowyo for the November 30, 1994 revision of the Identification of <br />Interests revision (MR-8). <br />One of the reasons [hat HGCC volunteered to compile the 'Consolidation' TR application, <br />which has been in preparation for some months now and is awaiting submittal pending approval <br />of MR-9, was to provide the DMG with a document which includes numbered, dated pages so <br />that future uncertainty regarding permit revisions can be avoided. We are pleased that the <br />Division has agreed to accept permit documents with file annotation "footers" as we feel tha[ <br />they serve as extremely useful document identifiers. <br />8. As discussed with you and Erica Crosby, one of the purposes of the secretarial certificates <br />provided as an addendum to the MR-9 submittal was to provide end dates for all directors and <br />officers no longer working for HGCC or its corporate owners. While we feel that the <br />secretarial certificates previously provided serve that function and fully satisfy the AAVS <br />requirements, additional documentation is provided with this submittal regarding end dates for <br />Mssrs. Bailey and Locke in the interest of resolving this matter. We are also furnishing you <br />with original secretarial certificates showing start dates of current directors and executive officers <br />of \V.R. Grace & Co. and W.R. Grace & Co.-Conn. <br />HGCC looks forward to timely approval of MR-9 so that we can finalize the pending TR <br />application which will incorporate this information, as well as other information relevant to the <br />current mine status, as explained on our recen[ meeting and correspondence. <br />Sincerely, <br />TerraMatrix Inc. <br />~i~~1_ <br />Michael R.
