1 December 23, / 999 ~ ~ Page C-9 <br />C.4.2.3 Custody Seals <br />' Custody seals are preprinted adhesive-backed seals with security slots designed to break if <br />the seals are disturbed. Sample shipping containers (coolers, cardboard boxes, etc.) are sealed with <br />custody seals in as many places as possible to ensure security. Seals are signed and dated before use <br />and strapping tape should be placed over the seals to ensure that seals are not broken accidentally <br />' during shipping. Upon receipt at the laboratory, the sample custodian must check (and certify, by <br />completing the package receipt log) that the seals on coolers, boxes, and bottles are intact. <br />' C.4.2.4 Chain-oj-Custody Form <br />The chain-of-custody (COC) record will be completed, placed in a plastic bag, and taped to <br />the inside lid of the sample cooler. Instructions for filling out the COC are given in Section C.5.3. <br />C.4.2.5 Sample Skipment <br />' Samples should be shipped every day during sampling. Samples will be shipped in iced <br />coolers with a completed chain-of-custody form taped to the inside of the cooler lid. Coolers will <br />' be sent to the lab by standard overnight delivery orhand-carried to aColorado-certified laboratory. <br />' The choice of laboratory could change during the course of monitoring program. <br />Samples will be shipped such that the holding times are not exceeded. <br />' C.5 DOCUMENTATION <br />' C.5.1 Field Sampling Data Sheet <br />A field sampling data sheet for groundwater samples should be completed at each sampling <br />location. The data sheet will be filled incompletely. If items on the sheet do not apply to a specific <br />location, the item will be labeled as not applicable (NA). The following information is included on <br />' the data sheet: <br />Well number <br />' ~ Date and time of sampling <br />Person(s) performing sampling <br />' ~ Volume of water evacuated before sampling <br />Conductivity, temperature, and pH during evacuation <br />' ~ Time samples are obtained <br />' ~ Number of samples taken <br />Sample identification number <br />' Agapito Associates, Inc. <br />
