The due date for the <br />pnmittte's romplianoe report will be listed in their permit rnrificarion. The Division resers+es the right to <br />require additional information in the repots, and more frequent reports, on a case-by-case basis, as needed. <br />All reports required for submittal shall be signed and rnt~ed for accuracy by the permittee. (See Part I.D.9 <br />of the permit.) <br />G. Mon_ i <br />Sampling and resting of stormwater for specific parameters is not required on a routine basis under this permit. <br />However, the Division resares the riglu to require sampling and testing, on a case-by-case basis, in the n+em that <br />there is reason to suspect that rompliasce with the SWMP is a problem, or to measure the effectiveness of the <br />BMPs in removing pollutams in the effluem. <br />H. Additional Submittal Unon Reauut <br />Upon request, the permittee shall submit the SNMP to the Division, EPA, the Colorado Division of Minerals and <br />Geology, and any lorol agency approving sediment and erosion plans, or storntwater management plans. <br />Retention of Rerords <br />The permittee for active mining operations slut!! strain a ropy of the SR'MP required by this permit at the mining <br />site for the duration of this permit. 7ite permittee jot inauirt mating operations shall retain a mpy of the SWMP <br />required by this permit, with the local rontact listed in the application for the duration of this permit. The <br />perminee steal/ retain ropier of all other rrports rcquinad by this pmnit erred rcrords of all data used to complete <br />the application to be rovered by this permit, jot a period of eu least three years. <br />J.- Inactivation Notice <br />When a site has been reclaimed in accordance with the Colorado Division ojMinerals and Geology requirements <br />and the performance bond has been released, the operator of the facility shall submit an Inativation Notice with <br />proper docrunemation, that is signed in acrordmtoe with Parr LD.9 of the Pnmir. The rontems of the Inactivation <br />Notice are described in Part II.B.4.d cf the permit. <br />Continued rot+erage may be required after reclamation is complete if the Division has shown or has reason ro <br />suspect that the stormwater discharge may rontributt to a violation of a water quality standard. <br />K. Duration of Permit <br />The general permit shall not esoeed, five years in duration. The permittee's authority to disdutrgt under this permit is approved until the expiration date of <br />the general permit.
