STORMWATER MANAGEMENT PLAN -ACTIVE MINES (watt <br />The plan shall identify potential sources of pollution (including sediment) which may reasonably be expected to affect the quality <br />of stormwater discharges associated with tk~e mining operation. In addition, the plea shall describe and erasure the <br />implementation of best management practices (BMPa) which wiB be used W reduce the poUutaats in stormwater discharges <br />associated with mining activity. <br />As a condition of this permit, active mining operations must ceRify the wmpletion and implementation of the SWMP required <br />under this part. Aetive mining optxatioas must ®pl®eot the provisions of their SW11~II' m e condition of this permit. <br />The Division reserves the right to request awl review the plans, and to require additional measures to prevent and control <br />pollution, m needed. <br />The SWMP shall include the following items, at a minimum: <br />Description of Minirr¢ Activities <br />The plea shall provide a narrative description of the mining and associated activities taking place at the site which affect <br />or may affect stormwater runoff intended to be wvered by this permit. The narsative description shell report the total <br />acreage within the mine site and an estimate of the number of acres of disturbed area. A general description of the <br />location of the mining site relative to major transportation routes and wmmunities shall also be provided. <br />2. Arne Subject to Effluent Limitations Guidelines <br />Each plan shall describe the portion of the mining site which wntribute nmoff to areas subject to effluent limitations <br />guidelines contained in 40 CFR subchapter N. <br />3. 'te Ma <br />Each plea shall provide a site map or maps which indicate at a minimum: <br />- mining/milling site boundaries and access sad Gaul roads; <br />- equipment storage. fueling sad maintenance cram; <br />- materials handling cress; <br />- areas used for storage of overburden, materials, soils or wastes; <br />- areas used for outdoor mmufacturing, storage or disposal of materials; <br />- location of mine drainage or any other process water; <br />- tailings pilrs/poads, both new and pre-existing: <br />- mine dnimge or any other process water discharge points; <br />-.- - springs. streams, wetlands sad other surface waters: and <br />°• - boundary of area that contributes nmoff to outfalls that are subject to effluent limitations guidelines. <br />For those areas outside of the area that contributes runoff to outfalls that are subject to effluent limitations <br />guidelines: <br />'- - an estimate of the direction(s) of flow; <br />- stormwater outfalls and an outline of the drainage errs of each stormwater outfall; <br />- each existing structural wntrol measure to reduce pollutants in stormwater runoff: end <br />the location of impervious structures (e.g. panting lots, roofs, etc.) <br />4. Description of Potential Pollutant Sor~/Materisl Inventors <br />The plan shall provide a description of potential sources which may reasombly be expected to add pollutants to <br />stormwater discharges. The plan shall identify all activities and materials which may potentially be pollutant soumxs. <br />This description of sources should agree with sources identified ao the map. An inventory end narrative description of mgaificant materials (see definitions is Part I.D. of this permit) that have <br />bcea treated, stored or disposed of in a mercer to allow exposure to stotmwater from three years prior to permit <br />
