Sit!nalory Reuuiremenls <br />All reports required far submittal shall be signed and certified for accuracy by the perminee in accord with the <br />following criteria: <br />a) In the case of corporations, by a principal executive officer of at least the level of vice-president or his or her <br />duly authorived representative, if such representative is responsible for the overall operation of the facility <br />from which the dischazge described in the form originates; <br />b) In the case of a partnership, by a general partner; <br />c) I^ the case of a sole proprietorship, by the proprietor; <br />d) In the case of a municipal, state, or other public facility, by either a principal executive officer, ranking <br />elected official, or other duly authorized employee. <br />E. SPECIAL REQUIREMENTS <br />I. Materials Containment Plan <br />Pursuant to Sections 6.9.3 (~ and (6)(b) of the Regulations for the State Dischazge Permit System, the permittce is <br />required to submit a Materials Contaimnent Plan. Such a plan shall be submitted to the Permits and Enforcement <br />Section, Water Quality Control Division within ninety (90) days after the effective date of this permit and must be <br />implemented. The plan shall include information and procedures for the prevention and containment of spills of <br />materials used, processed or stared at the facility which if spilled would have a reasonable probability of having a <br />visible or otherwise detrimental impact on waters of the State 1/ 2/. The plan shall include, but not necessarily be <br />limited to: <br />a) A history of spills which have occurred in the three (3) years preceding the effective date of this permit. The <br />history shall include a causation of the spills and a discussion of preventative measures designed to prevent <br />them from reoccurring; <br />b) A description of the reporting system which will be used to notify responsible facility management, the State <br />Water Quality Control Division, the Environmental Protection Agency, downstream water users within 5 miles <br />downstream of the facility, and local health officials; <br />c) A description~of preventative facilities (including overall facility plot) which prevent, contain, or trrzt spills <br />and unplanned discharges; <br />d) A list which includes the volumes or quantities of all materials used, processed, or stored at the facility which <br />represent a potential spill threat to surface waters. The location of stored material shall be indicated on the <br />facility plot submitted for item c; <br />e) An implementation schedule for additional facilities which might be required in item c, but which are not yet <br />operational; <br />t) A list of available outside contractors, agencies, or other sources which could be utilized in the event of a spill <br />in order to clean up its effects. if the facility is capable of handling spills in-house, this shall be documented <br />in the plan; <br />
