All reports required for submittal shall be sigrted end certified for accuracy by the permittee in accordance with <br />the following criteria: <br />1) Tn the case of corporations, by a principal executive officer of at least the level of vice-president or his or <br />her duly authorved representative, if such representative is responsible for the overall operation of the <br />facility from which the discharge described in the form originates; <br />2) fn the case of a partnership, by a general partrrer; <br />3) In the case of a sole proprietorship, by the proprietor, <br />4) In the case of a municipal, state, or other public facility, by either a principal executive officer, ranking <br />elected official, or other duly authorized employee. <br />b. Chturges to authorization. If an authorization under paragraph a) of this section is no longer accurate because <br />a different individual or position has responsibility for the overall operation of the facility, a new authorization <br />satisfying the requirements of paragraph e) of this section must be submitted to the Division, prior to or together <br />with any reports, information, or applications to be sighed by m authorized representative. <br />a Certification. Any person signing a document under paragraph a) of this section (except for the SWMP <br />certification) shall make the following certification: <br />"I certify under penalty of law that this document and all attachments were prepazed under my direction <br />or supervision in accordance with a system designed to assure that qualified personnel properly gather <br />and evaluate the information submitted. Based on my inquiry of the person or persons who manage the <br />system, or those persons directly responsible for gathering, ttre information submitted is, [o the best of <br />my knowledge and belief, true, accurate, and complete. I am aware that there are significant penalties <br />for submitting false information, including the possibility of fine and imprisonment for knowing <br />violations.' <br />E. FOOTNOTES -APPLICABLE TO PRECEDING PAGES <br />a! The thirty (30) day average shall be determined by the arithmetic mean of all samples collected during a thirty (30) <br />consecutive-day period. Samples shall not be used for more than one (1) reporting period. <br />b! The seven (7) day average shall be determined by the arithmetic mean of all samples taken in a seven (7) day period. <br />Samples may not be used for more than one (1) reporting period. <br />c/ This limitation sball be determined by a single sample or set of samples as required by Part I.B.3, Sample Type. <br />d/ Monitoring is required only during periods of discharge. If no discbazge occurs, this shall be reported at the specified <br />frequency. (See Part I.D.3.) <br />e/ TDS monitoring is only required for facilities discharging in the Colorado River Basin. TDS may be measured as <br />electrical conductivity where a satisfactory correlation with TDS has been established based upon a minimum of five <br />(5) samples. See the individual Certification Rationale for a determination as to whether monitoring is required. <br />f/ Phosphorus monitoring is requited only for facilities which discharge into the following drainage basins: Cherry Creek <br />basin, Chatfield Reservoir downstream of the USGS gage au Waterton and on Pltrm Creek, Dillon Reservoir basin (i.e., <br />Teu Mile Crcek, Snake River, Blue River, ell tributaries to the Dillon Reservoir), and Bear Creek basin. See the <br />individual Certification Rationale for a determination as to whether monitoring is required. <br />
