a. All reports required for submittal shall be sigrted end certified for accuracy by the permittee in accordance with
the following criteria:
1) Tn the case of corporations, by a principal executive officer of at least the level of vice-president or his or
her duly authorved representative, if such representative is responsible for the overall operation of the
facility from which the discharge described in the form originates;
2) fn the case of a partnership, by a general partrrer;
3) In the case of a sole proprietorship, by the proprietor,
4) In the case of a municipal, state, or other public facility, by either a principal executive officer, ranking
elected official, or other duly authorized employee.
b. Chturges to authorization. If an authorization under paragraph a) of this section is no longer accurate because
a different individual or position has responsibility for the overall operation of the facility, a new authorization
satisfying the requirements of paragraph e) of this section must be submitted to the Division, prior to or together
with any reports, information, or applications to be sighed by m authorized representative.
a Certification. Any person signing a document under paragraph a) of this section (except for the SWMP
certification) shall make the following certification:
"I certify under penalty of law that this document and all attachments were prepazed under my direction
or supervision in accordance with a system designed to assure that qualified personnel properly gather
and evaluate the information submitted. Based on my inquiry of the person or persons who manage the
system, or those persons directly responsible for gathering, ttre information submitted is, [o the best of
my knowledge and belief, true, accurate, and complete. I am aware that there are significant penalties
for submitting false information, including the possibility of fine and imprisonment for knowing
violations.'
E. FOOTNOTES -APPLICABLE TO PRECEDING PAGES
a! The thirty (30) day average shall be determined by the arithmetic mean of all samples collected during a thirty (30)
consecutive-day period. Samples shall not be used for more than one (1) reporting period.
b! The seven (7) day average shall be determined by the arithmetic mean of all samples taken in a seven (7) day period.
Samples may not be used for more than one (1) reporting period.
c/ This limitation sball be determined by a single sample or set of samples as required by Part I.B.3, Sample Type.
d/ Monitoring is required only during periods of discharge. If no discbazge occurs, this shall be reported at the specified
frequency. (See Part I.D.3.)
e/ TDS monitoring is only required for facilities discharging in the Colorado River Basin. TDS may be measured as
electrical conductivity where a satisfactory correlation with TDS has been established based upon a minimum of five
(5) samples. See the individual Certification Rationale for a determination as to whether monitoring is required.
f/ Phosphorus monitoring is requited only for facilities which discharge into the following drainage basins: Cherry Creek
basin, Chatfield Reservoir downstream of the USGS gage au Waterton and on Pltrm Creek, Dillon Reservoir basin (i.e.,
Teu Mile Crcek, Snake River, Blue River, ell tributaries to the Dillon Reservoir), and Bear Creek basin. See the
individual Certification Rationale for a determination as to whether monitoring is required.
