The estimate of reclamation costs submitted with [he permit application <br />totals $9.921.00. According to calculations by the Division of Minerals and Geology, the total estimate <br />is $ ] 3,502. See the attached calculation sheets. <br />Rule 6.3.5 Exhibit E -Map <br />8. Exhibit E is defined as the Map. (b) Label the names of owner(s) of record of the surface of the affected <br />area and of the land within two hundred feet of the affected area. Please indicate the owner of the <br />property immediately adjacent to the southwest portion of the affected area. (c) The Mining Plan Map is <br />to show the location(s) of stockpiled topsoil. Please add the above information to the Mining Plan Map. <br />(d) The Mining Plan Map is to indicate [he direction that construction material extraction will occur. <br />Please add the above information [o the Mining Plan Map. <br />Additionally, per our conversation of this date, [he Division needs proof of publication of notice concerning <br />the intended activities at the site. <br />Please be advised that the 3B Enterprises New 110 Permit Application may be deemed inadequate, and [he <br />application may be denied on July 30, 2001 unless the above mentioned adequacy review items are <br />addressed to the satisfaction of [he Division. If you feel more time is needed to complete your reply, the <br />Division can grant an extension to the decision date. This will be done upon receipt of a written waiver of <br />your right to a decision by July 30, 2001 and request for additional time.
