Exhibit D is to describe the nature and thickness of the mined as well as the type of overburden to be <br />removed. Please specify the above information. <br />Rule 6.4.5 Exhibit E -Reclamation Plan <br />6. Exhibit E is defined as the Reclamation Plan. The Reclamation Plan is to compare of the proposed <br />post-land use to other land uses in the vicinity. Please supply this information. <br />7. The Reclamation Plan in the original application includes Crested Wheatgrass in the seed mix to be <br />planted at the site. Although this species was widely used at the time of the original permit <br />application, it has since been deemed less than desirable in a reclamation seed mix. It is <br />recommended that the seed mix for the reclamation be reassessed. Additionally the original plan <br />indicates that native shrubs will be included in the revegetation of the site. Please specify what <br />species will be included. <br />Rule 6.4.12 Exhibit L -Reclamation Costs <br />8. Exhibit L is defined as Reclamation Costs. Calculated reclamation cost in the original permit <br />application is listed as $103,600. The costs to reclaim the will be recalculated by the Division. <br />Some of the information contained in this adequacy review letter is necessary prior to reclamation <br />cost calculation. Please supply the information requested. <br />Rule 6.4.13 Exhibit M -Other Permits and Licenses <br />9. Exhibit M is defined as Other Permits and Licenses. Exhibit M is to identify permits, licenses and <br />approvals held by the Operator/Applicant, or those that will be sought by the Operator/Applicant in <br />order to conduct the proposed mining and reclamation operations. Please supply this information. <br />Rule 6.4.16 Exhibit P -Municipalities Within Two Miles <br />10. Exhibit P is defined as Municipalities Within Two Miles. Exhibit P is to list any municipalities <br />within two miles of the proposed mining operation and address of the general office of each <br />municipality. Please supply this information. <br />Rule 6.4.17 Exhibit Q -Proof of Mailing of Notices to Board of County Commissioners and Soil <br />Conservation District <br />11. Exhibit Q is defined as Proof of Mailing of Notices to Board of County Commissioners and Soil <br />Conservation District. Exhibit Q is to show proof that notice of the permit application was sent to <br />the Board of County Commissioners and if the mining operation is within the boundaries of a Soil <br />conservation District, to the Board of Supervisors of the local Soil Conservation District.
