1. Name of the operator conducting the blast. <br />2. Location, date, and time of the blast. <br />3. Name, signature, and license number of blaster-in-charge. <br />4. Direction and distance, in feet, to the nearest building within +k mile of the blasting site <br />either: <br />a. not located in the permit area. <br />b. Not owned nor leased by the person who conducts the surface coal mining operation. <br />5. Weather conditions, including temperature, wind direction, and approximate velocity. <br />6. Type of material blasted. <br />7. Number of holes, burden, and spacing. <br />8. Diameter and depth of holes. <br />9. Type of explosives used. <br />10. Total weight of explosives used. <br />11. Maximum weight of explosives detonated within any 8-millisecond period. <br />• 12. Maximum number of holes detonated within any 8-millisecond period. <br />13. Initiation system. <br />14. Type and length of stemming. <br />15. Mats or other protection used. <br />16. Type of delay detonator and delay periods used. <br />17. Sketch of the delay pattern. <br />18. Number of persons in the blasting crew. <br />19. Seismographic records, where required. <br />6.34 The maximum peak particle velocity in blasting operation will not exceed linch-per-second. All <br />blasting charges will be sized in accordance with the equation W=(D/60)2, where W= the <br />maximum weight of explosives in pounds, that can be detonated in any 8-millisecond period, <br />and D= the distance in feet, from the blast to the nearest dwelling, school, church, or commer- <br />cial or industrial building. <br />VII. Post-Shot Area Examined <br />7.1 The blasted area shall be examined for undetonated explosives immediately after each blast but <br />not until after smoke and dust have dissipated. <br />. 7.2 Misfires (holes with unexploded explosives) shall be disposed of by qualified and authorized <br />persons before any other work is performed in the blast area. <br />
