PERMIT NUMBER: COG-850000 <br />MONITORING FREQIIENCIES: (Continued) <br />As described earlier, the permittee is required to monitor all parameters, <br />regardless of whether precipitation ezemptiona are sought. The permittee must <br />submit an ezemption request and supporting 'proof' with the monitoring data. <br />The division will approve or deny the exemption request. If denied, then the <br />permittee will be expected to meet the primary parameters (TSS and total iron <br />for active mining, settleable solids for poet--mining). <br />REPORTING: <br />Discharge monitoring reports (DMRs) are to be submitted on a quarterly basis. <br />If no discharge occurred during the quarter, the DMR should state "No <br />discharge during this reporting period." See Part I.C of the permit for <br />additional requirements. <br />MATERIALS CONTAINMENT: <br />As most facilities provide bulk storage of some volume of petroleum products <br />or other chemicals, the permit will require adequate protection of such <br />facilities so as to prevent loss of these materials into state waters. Such <br />protection can take various forms, however, diking in most cases will prove to <br />be the mast cost effective. This provisioc is required as the division <br />interprets proper operation as properly addressing potential pollutaat sources <br />before problems occur. <br />CHANGES IN DISCHARGE POINTS: <br />M <br />Any time the permittee wishes to change the location of an outfall, add or <br />deactivate outfalls, or change an oui~fall from active to post~ining; it will <br />be necessary for the permitted to submit an amendment request.
