Page 117 <br />Hard Rock/Metal Mining Rule 6 <br />6.3.8 EXHIBIT H -Municipalities Within aTwo-mile Radius <br />List the mailing address and telephone number of the governing body for all <br />municipalities within a 2-mile radius of the proposed mining operation. <br />6.3.9 EXHIBIT I -Proof of Feline with County Clerk <br />An affidavit or receipt indicating the date on which the application was placed with <br />the local County Clerk and Recorder for public review, pursuant to Subparagraph <br />1.6.2(1)(c). <br />6.3.10 EXHIBIT J -Proof of Mailing of Notices to Board of County <br />Commissioners and Soil Conservation District <br />Proof that notice of the permit application was sent to the Bord of County <br />Commissioners and, if the mining operation is within the boundazies of a Soil <br />Conservation District, to the Board of Supervisors of the local Soil Conservation <br />District, pursuant to Subpazagraph 1.6.2(1)(a)(ii). <br />6.3.11 EXHIBIT K -Reserved <br />6.3.12 EXHIBIT L -Permanent Man-Made Structures <br />The Applicant shall provide information sufficient to demonstrate that the stability of <br />any structures located within two hundred (200) feet of the operation or affected land <br />will not be adversely affected. If the Office determines that such information is <br />inadequate to demonstrate that the operation will not adversely affect the stability of <br />any significant, valuable or permanent man-made structure, the Applicant shall either: <br />(a) provide a notarized agreement between the applican[ and the <br />person(s) having an interest in the structure, that the applicant is to provide <br />compensation for any damage to the structure; <br />(b) where such an agreement can not be reached, the applicant <br />shall provide an appropriate engineering evaluation that demonstrates that <br />such structure shall not be damaged by activities occurring at the mining <br />operation; or <br />(c) where such structure is a utility, the applicant may supply a <br />notarized letter, on utility letterhead, from the owner(s) of the utility that the <br />
