Glacier Peak Mining <br />Joe Dorris, Owner. <br />Plan of Operations - 06/13/2000 <br />VI. FOREST SERVICE EVALUATION OF PLAN OF OPERATIONS <br />Continuance of FS-2800-5, Plan of Operations for Mining Activities on NFS lands <br />A. Required Changes/modifications/special mitigation for plan of operations: <br />This plan will replace the previous Plan of Operation, submitted on August 25, 1993. In Reference to the Plan of Operation <br />submitted by Joseph Dorris on June 13, 2000: <br />Sanitation: All [rash will be hauled off National Forest System lands. Human waste will be disposed of by followine "Leave <br />No Trace" guidelines: <br />A "cathole" should be dug in the [op of organic soil and be at least 200 feet way from camp, trails, and water sources. <br />Groups may need to walk well over 200 feet to ensure that catholes are scattered during their stay at the site. <br />HAZMAT: Any type of fuel or lubricant will be stored off-site and will remain in the bed of vehicles while working the <br />claim. In the event of a petroleum (gas, diesel, oil, etc.) spill or other hazazdous material spill onto [he soil of the claim, <br />immediately contact [he Park County Sheriff. If hazardous substances aze spilled on a federal, state, or county roadway contact <br />[he Colorado S[a[e PaVOI. All hazardous material spills will be disposed of according to Colorado State Hazmat disposal <br />guidelines. <br />Final reclamation will include: I) removing all equipment and debris from the claim and properly disposing items off NFS <br />lands, 2) refilling all pits and trenches, 3) re-contouring the slope to the natural setting and to minimize erosion, 3) the <br />replacement of available topsoil, 4) closing all non-system access roads, and 5) reseeding the disturbed area with native <br />vegetation. All disturbed areas on the claim are to he reseeded with South Park RD recommended native vegetation and tree <br />species. Before reclamation bond is released, vegetation will be established 50% or better compared to the surrounding <br />undisturbed areas. Reclamation of the site will be measured by the Minerals Administrator, South Park RD. <br />Bonds: Currently a 900 bond is being held for this claim. The bond amount will be reviewed annually and adjusted to <br />compensate for completed reclamation work, changes in equipment rental rates, increased scope of operations, etc. After re- <br />establishing the site according to the site reclamation plan, the release of bond criteria will be as follows: 1) 50%o of vegetation <br />is present compared to undisturbed vegetation immediately adjacent to the mining site, and 2) all equipment and debris are <br />removed from the site. <br />TERMS AND CONDITIONS: <br />A. If a bond is required, it must be furnished before approval of plan of operations <br />B. Information provided with this plan marked confidential will be treated in accordance with the agency's laws, rules, and <br />regulations. <br />C. Approval of this plan does not constitute certification of ownership to any person named herein and/or recognition o the <br />validity of any mining claim herein. <br />D. Approval of [his plan does no[ relieve the claimant of [heir responsibility to comply with state or federal laws, rules, or <br />regulations. <br />E. If previously undiscovered cultural resources (historic or prehistoric objects, aztifacts, or sites) are exposed as a result of <br />operations, those operations will no[ proceed until notification is received from the Authorized OfScer that provisions for <br />mitigating unforeseen impacts as required by 36 CFR 228.4(e) and 36CFR 800 have been complied with. <br />F. This plan of operations has been approved (or a period of 3 seasons or until December 31, 2003. Anew or <br />revised plan must be submitted in accordance with 36 CPR part 228, subpar[ A, if operations arc to be continued after that time <br />period. <br />Revisions made by: <br />Megen D.
