Rule 11 <br />Permits, Licenses and Approvals <br />Foao~~i <br />1. Placer claims for chemical grade limestone have been properly located <br />on the subject lands. The certificates of location have been timely <br />filed with Moffat County and the BLM. <br />2. A Notice of Intent has been filed with the BLM. Inasmuch as land dis- <br />turbance on BLM lands will not exceed five acres per year, no permits <br />are required from the BLM. <br />3. A Notice of Operations and a Legal Identity Report will be filed with <br />MSHA in July, 1982. <br />4. An Application for a permit to use explosives will be applied for to <br />BAFT in July, 1982. <br />5. An Explosive Materials Permit will be applied for to the Department of <br />Labor in July, 1982. <br />State of Colorado <br />1. An Air Pollution Emission Notice will be submitted to the Colorado Department <br />of Health (APCD) in July, 1982. <br />2. Applications for Fugitive Dust and Process Permits will be filed with the <br />Colorado Department of Health (APCD) in July, 1982, if necessary. <br />3. A Notice of Activity was submitted in June to the Colorado Department of <br />Mines. <br />4. An Application to store and use explosives will be filed with the Department <br />of I.1ines in August, 1982. <br />5. A diesel permit will be applied for to the Department of Mines in August, 1982. <br />Moffat County <br />1. An Application for a Conditional Use Permit covering the quarry site and <br />the processing site will be submitted in July, 1982. <br />2. Applications for sewage permits for the quarry and processing sites will be <br />submitted to the Planning Office in July, 1982. <br />3. Applications for building permits for the installation of crushing equipment <br />and other aspects of the processing site will be submitted to the Planning <br />,j Office in July or August, 1982. <br />
