Nancomollaaee Notification <br />a) If, for nay reason, the permittee does not comply With or will <br />be unable to comply with any discharge limitations or standards <br />specified is this permit, the permittee shall, at a minimum, <br />provide the {Dater Quality Control Division and EPA with the <br />following information: <br />(1) A description of the discharge and cause of noncompliance; <br />(11) The period of noncompliance, including ezact dates and <br />times and/ar the anticipated time when the discharge will <br />return to compliance; and <br />(iii) Steps being taken to reduce, eliminate, and prevent <br />recurrence of the noncomplying discharge. <br />b) The permittee shall report the following iastaaces of <br />noacompliaace orally within tveaty-four C24) hours from the time <br />the permittee becomes aware of the noacompliaace, and shall mail <br />to the Division a written report within five CS) days after <br />becoming awaze of the noacompliaace: <br />(i) Aay instance of noacompliaace which may endanger health <br />or the environment; <br />(Si) Any unanticipated bypass; <br />(iii) Any upset which causes an ezceedance of any effluent <br />limitation is the pec:ait; <br />Civ) Aay spill which causes nay effluent limitation co be <br />violated; <br />(v) Daily mazimum violations foc nay cozic pollutants or <br />hazardous substances limited by PART I-A of this permit <br />sad specified as requiring 24 hour aotificacioa. <br />c) The permittee shall report all other iastaaces of non-compliance <br />not requiring 24-hour notifieatloa at the time Discharge <br />Monitoring Reports are submi[ted.
