6. Industrial'Aelivity: Describe the primary industrial activities which take place on site. Include dte type of facility (underground mine,
surface mine, err.) plus a brief description of the nature of the business and the industrial processes used. Include a description of the mining
and coal preparation processes where applicable. (The applicant may want to submit a process flow sheet.)
Carbon .Junction ?tine is a surface nine wlriclt, when active, n_ roduces coal for sale and
[ransnor[, At this time the mine is in temnorarv cessation, with the only activities
beinC placement of waste material from a nearby gravel operation (nermittecl) on a mine
7. Indicate whether an exploratory, active mining, inactive mining, or post mining operation or is the site in temporary cessation.
The site is in temporary cessation.
1t exploratory, plnx submit any known water qualirylquantiry dau relative to the discharge(s) and the receiving stream(s) that reflect the
conditions prior to the present activity, the length of tune the activity is expected to be under exploration and describe what activities will
lake place during exploration that could have an impact on the quantity or the quality of the discharge. if post mining, describe arcas
affected and extent of «clamation completed and length of time elapsed since revegeution occurred. Also, specify whether any outfalls
previously described serve, partially or totally, a reclaimed area. Include expected bond release date.
V.A.
8. Production: List the principal product(s) produced and tumor and maximum production rates.
The yrincipal product is coal, The currenC production rate is zero tons per day.
9. $ this a seasonal operation? No ~ Yes ^ If Yes, please list the months of operation.
If coal were beine produced it would not be on a seasonal basis.
10. Intermittent Discharges: Except for storm runoff, are any of the discharges described in item 21 intermittent or seasonal?
No ^ Yes ^ If Yes, describe the frequency, duration and Bow rate of each discharge occurrence.
*I.A. Storm runoff only,
11. l.otalion Map: A g'h : 11 Inch location map designating the facility property, intake points, discharge points, each of its hazardous waste
treaunent storage or disposal facilities, areas in post mining, each well where fluids from the facility are injected underground, those wells,
springs, odter surface water bodies and drinking water wells listed N public «cords or otherwise known to the applicant and the receiving
waters shall be submined. The map shall extend one mile beyond the property boundaries. The map shall be from a 7'h or IS minute USGS
quad sheet, or a map of comparable scale. A north arrow shall be shown. A larger map may be included for reference if the permittee
wishes. See Exhibit A.
12. Site Map: A 8'fi x 11 inch map of the site shall be submitted showing facility features (whenever possible) such as buildings, ponds,
diversion ditches, stream locations, numbered discharge points, sampling and flow monitoring points, land application areas, stockpiles, waste
rock piles, topsoil piles, location of french drains andlor spoils springs, trosher areas, mine drainage Bow paths, domestic wastewater
treatment plants, power plants, wck washing areas, explosive storage areas, parking lots, vehicle maintenance areas, chemical storage areas,
etc. Thr: outfalls shall be labeled to correspond with the numbers listed in item 21. A larger map may be included for reference if the
permiltee wishes.
See E:chibit 3.
13. Water Balance: Attach a line drawing showing all water Bow through and from the mine/preparation plant sites. Indicate sources of intake
water, operations convibuting wastewater to the effluent, and treatment units labeled to correspond to the more detailed descriptions in item
21. Construct a water balance on the line drawing by showing average Bows between inukes, operations, treatment units and outfalls. If a '
water balance cannot be determined, provide a pictorial description of the nature and amount of any sources of water and any collection or
veaunent measures. 1f not applicable, please indicate. N. A. No process water used.
