Bruce Addington <br />Lorencito Coal Company, LLC <br />9/23/2004 <br />Page 2 <br />The recommendations will enable your system to better conform to the requirements of applicable <br />design criteria or other industry standards: <br />4. On August 4, 2004 approximately 4.4 inches of rain reportedly fell in the area and caused <br />considerable damage to some of the reclamation efforts. Repairs must be commenced as soon as <br />possible to prevent additional damage to the water conveyance structures. <br />5. The erosion gully formed on the embankment for Outfall #008 during the August 4, 2004 storm <br />event must be repaired to ensure the structure will operate as designed. <br />6. The storm event of August 4, 2004 sepazated the v-notch weir from the effluent piping at Outfall <br />#009. The effluent piping must be repaired and the v-notch weir replaced so that accurate flow <br />readings can be obtained for reporting purposes. <br />7. The storm event of August 4, 2004 caused the v-notch weir for Outfall #007 to settle unevenly. The <br />v-notch weir must be leveled so that accurate flow readings can be obtained for reporting purposes. <br />8. The Settleable Solids sampling results for Outfa11009A were reported as >5 mUl on the 9130/03 <br />Discharge Monitoring Report (DMR), which exceeds the max effluent limitation of 0.5 mUl <br />contained in the discharge permit. It is recommended that the original sample results be verified to <br />determine if this is the correct result. If the Settable Solids results were reported in error, the 9/30/03 <br />DMR must be corrected to reflect the correct result, and a copy of the amended document <br />resubmitted to the Division's Denver office at the same address to which the routine DMRs aze sent. <br />Attached to this letter you will find a Customer Satisfaction Survey.
