Deaz Mr. Addington:
This letter serves to report the results of the compliance inspection conducted by the Technical Services
Unit of the Water Quality Control Division ("the Division") on the above-referenced facility on August
17, 2004. The assistance of Ron Thompson was very helpful and greatly appreciated. I have enclosed a
copy of the inspection report for your records.
Violations•
1. No violations were noted.
Permit Issues:
These items may assist the Division during the next dischazge permit renewal or to provide clarification
for an amendment to the current permit. During the inspection, we observed the following facility
conditions that aze not accurately represented in the permit:
2. The information on the legal contact listed on the discharge permit is no longer current. To amend
the permit to reflect the new legal mailing addresses, and telephone numbers, the Division must be
contacted, in writing, of the change.
3.. The facility has ceased active mining and has started reclamation efforts. The mine has applied for
Phase 1 bond release from the Colorado Division of Minerals and Geology. The Division of
Minerals and Geology is currently reviewing the application.
