Application Package
Dear Mr. Phillips:
On June 16, 2000, the Division of Minerals and Geology received your 112 Construction Materials
Reclamation Permit Application package for the Pioneer Farm, File No. M-2000-080,. Preliminary review of
the information received determined that the following items must be received before the Division can consider
your application as being submitted and technical review can begin:
APPLICATION FORM: Page 1, Item 1: The Applicant/operator or company name (name to be
used on mermitl is listed as Robert Allen Grimm. Page 3, Item 14: ApplicandOperator (Operator
name is circled) (name, address, and phone of name to be used on nermitl is listed as Pioneer Sand
Company, Inc. These two areas must be consistent. !f Mr. Grimm is to be the Permittee then his name
should be listed on Page 1, Item 1 and his name, address, and phone number should be listed on Page 3,
Item l4 ApplicandOperator area. If Pioneer Sand Company, Inc. is a contrail operator for Mr. Grimm
you can list them under the Inspection Contact area if Mr. Grimm does not want to be informed when
an inspection is to be done or know the resuhs of an inspection. If he does want to know when an
inspection is going to be done and he wants to know the resuhs you will need to put his name, address,
and phone number in the Inspection Contact area. If Pioneer Sand Company, inc. is to be the permittee
Page 3 does not need to be changed but Page 1 should be changed. 'This information must also be
corrected at the Arapahoe County Clerk and Recorder's Office. The Applicant/Operator name must be
consistent on alt documents.
Authorization Letter: The letter, which you submitted from Mr. Grimm, does not authorize you as
his agent, it does allow you on his property to obtain information and measurements necessary for
obtaining a permit from Arapahoe County. If you are to represent Mr. Grimm or Pioneer Sand
Company, Inc. the letter will need to be redone for the application to the State of Colorado, Division of
Minerals and Geology.
EXHIBIT N -Source of Leal Rieht to Enter (Rule 6.4.14): If Mr. Grimm is going to be the
ApplicanUOperator (Permit holder) You must provide a copy of either his deed, abstract of title, or a
current tax receipt. If Pioneer Sand Company, Inc. is going to be the ApplicandOperator (permit
holder) then the Agreement found in Exhibit N will then be the right information.
