Dear Mr. Striggow,
The Division of Reclamation, Mining and Safety (Division) has completed its review of your responses to our initial
adequacy review letter. Listed below aze additional adequacy review comments on the proposed Glade Quarry Permit
Application No. M-2006-021. The Division is required to make an approval or denial decision no later than November
10, 2006; therefore, your response to the following adequacy review concerns should be submitted to the Division as soon
as possible.
Rule 6.4.3 Exhibit C -Pre-Mining Plan
1. The Pre-Mining Plan Map shows what appear to be several stockpiles of material east of the affected land boundary.
Unless these stockpiles actually exist, they should not be shown on this map. In addition, this reap shows what appear
to be reclamation contours and the proposed phases of mining. It is not necessary to show this information on this
map, and these items make this exhibit confusing. Please revise this map to show current site conditions.
Rule 6.4.4 Exhibit D -Mining Plan
2. Upon further review of the two maps included with this exhibit, it was determined that the boundary of the affected
area does not include all of the area that will be affected by mining. Specifically, all stockpiles, facilities (fuel storage
area, mobile building, scale, and fence), and access road must be included in the affected azea. Please revise both
maps in this exhibit to include the aforementioned items and revise the mining plan to reflect the increase in affected
area. Please be advised that increasing the affected azea will trigger an amendment to the permit application which, in
turn, will require the applicant to publish and mail new notice of the application in accordance with Rule 1.6.6.
Rule 6.4.6 Exhibit F -Reclamation Plan Man
3. Please revise the Reclamation Plan Map to reflect changes to the affected area that will be made as a result of
addressing Item No. 2 above.
