<br />described in Table 3, CC&V shall undertake the following steps: <br />a. Notify the OMLR by telephone within 3 working days of having knowledge of the <br />combination of elevated water-quality parameters. <br />b. If the concentration of CNW„p and pH listed in Table 3 are exceeded for more than 90 days, <br />or if the levels rise to <_ 1.0 mg/1 CNv,,,p and 59.5 s.u. pH, CC&V will consider there to be <br />a defect in the active Cresson Valley Leach Facility that allows CN-bearing solution from that <br />facility to move into the surrounding environment and will combine this information with the <br />information garnered from the LDS and the Underdrain system (see Protocols _ above) to <br />comply with Rule 8.0. including notification of the OMLR within 24 hours of CC&V having <br />knowledge of exceedences, a discussion of precautions and corrective actions undertaken and <br />planned br CC&V, and a written report satisfying Rule 8.2.3. Corrective actions will include <br />a specific plan of corrective action to be timely implemented by CC&V to control, reduce, or <br />otherwise minimize movement of leach solution in harmful quantities moving outside the permit <br />area. Though any such circumstance will necessitate case-specific measures, CC&V will address <br />testing of areas through manipulation of leach solution application. (?At any time when <br />redeployment of applied water has not mitigated the migration of leach Fluid from the pad to the <br />environment, CC&V will cease addition of additional water to the pad.?] An exception to the <br />this protocol will be allowed the CNWAp in ground water is demonstrated to originate from <br />outside of the valley leach facility. <br />d. The results of laboratory analyses for the listed parameters will be provided to the OMLR <br />along with the sampling locations, date and time of sampling, type of analyses, and applicable <br />laboratory sample number. <br />e. In the event water removed from wells exceeds the criterion listed in Table 3, the water <br />removed will either be returned to the well or, in view of the small amount involved, placed on <br />the Valley Leach facility.
