PART I <br />Page 6 <br />PERMIT NO.: COG-500000 <br />C. MONITORING AND REPORTING - <br />1. Representative Sampling <br />Samples and measurements taken as required herein shall be <br />representative of the volume and nature of the monitored discharge. <br />2. Reporting <br />Monitoring results far each one pear period COctober 1 - September 30) <br />are due by the 28th of October of each year or upon termination of <br />discharge. The results shall be summarized and reported on applicable <br />discharge monitoring report forms (EPA Form 3320-1), postmarked no <br />later than the due date. If no discharge occurs during the reporting <br />period "No Discharge" shall be reported. The report submitted at <br />termination of discharge shall be marked as a "final" report. <br />Duplicate signed copies of these, and all other reports required <br />herein, shall be submitted to the Regional Administrator and the State <br />at the following addresses: <br />Colorado Department of Health <br />Water Quality Control•Division <br />4210 East 11th Avenue Raom 200 <br />Denver, Colorado 80220 <br />Attention: Permits and Enforcement <br />U. Environmental Protection Agency <br />One Deaver Place <br />Suite 1300 <br />999 18th Street <br />Denver, Colorado 80202-2413 <br />Attention: Enforcement-Permit Program <br />Failure to submit the reports shall constitute a violation of the <br />permit and may constitute grounds for revocation of the permittee's <br />authority to discharge under the permit. <br />3. Defiaitioas <br />a) A "composite" sample, for monitoring requirements, is defined as a <br />minimum of four (4) grab samples collected at equally spaced two <br />(2) hour intervals and proportioned according to flow. <br />b) A "grab" sample, for monitoring requirements, is defined as a <br />single "dip and take" sample collected at a representative point <br />in the discharge stream. <br />c) An "instantaneous" measurement, for monitoring requirements, is <br />defined as a single reading, observation, or measurement. <br />d) A "continuous" measurement, for flow monitoring requirements, is <br />defined as using an automatic recording device to continually <br />measure flow. <br />
