PART I Page No. 12 Perini[ No. COG-850000 D. GENERAL MONITORING, SAMPLING AND REPORTING REQiJIREMENTS 1. Routine Reoortine of Data Reposing of the data gathered in compliance with Part I.B.3. shall be on a quarterly basis. Repotting of all data gathered shall comply with the requirements of Part I.D. (General Requirements). Monitoring results shall be s++mmari~rtl for each calendaz quarter and reported on Division approved discharge monitoring report (DMR) fotms (EPA form 3320-1). The forms shall be mailed to the agencies listed below so they are received no later than the 28th day of the month following the end of the quaver (for example, the DMR for the fast calendar quarter must be received by the Division by April 28th). If no discharge occurs during the reporting period, 'No Dischazge' shall be reposed. The DMR forttts consist of four pages -the [op "original" copy, and three attached nocarbon-required copies. After the DMR form has been filled out and signed, the four copies must be separated and distributed as follows: The fits[ o~inal signed copy of each discharge monitoring repon (DMR) shall be submit[ed to the Division at the following address: Colorado Department of Public Health and Environment WQCD-PE-B2 4300 Cherry Creek Drive South Denver, CO 80222-1530 The US Environmental Protec[ion Agency Region VIII no longer requires submittal of duplicate copies of the DMR to their office, therefore, the second, third and fourth copies are for the permittce records. The Discharge Monitoring Report forms shall be filled out accurately and completely in accordance with requirements of this permit and the instructions on the forms. Thry shall be signed by an authorized person as identified in Part I.D.7. Calculations for all limitations which require the averaging of measurements shall utilize an arithme[ic mean unless otherwise specified by the Division is the permit. 2. Reoortine - Stotmwater Annual Reoort The permittce will be required to submit an Annual Report, covering January 1 through December 31 of each yeaz, on the overall compliance with the SWMP. The Annual Repos will contain, at a minimum: a) Name of permit[ce, address, phone number, and permi[ certification number. b) A report on the facility's overall compliance with the SWMP. c) A summary of each comprehensive stormwater facility inspection made, including date, findings, and action taken. d) Results and interpreta[ion of any stormwater monitoring performed. e) The report shall be sighed and certified for accuracy by the petmittce, including ilte certification language contained in Pan I.D.7. of the permit. The Annual Repos will be due to the Division on or before February 15 of each yeaz, after the first full yeaz of coverage under the permi[. The exac[ due date for the permittce's fast Annual Repos will be listed in their permit certification. The Division reserves the right to require additional information in the report, on a case-by-case basis, as needed. All reports required for submittal shall be signed and certified for accuracy by the permittce (see Pan I.D.7.).
