At this time however if you have questions about the t es of <br />bond instruments accepted by the Division. or need bond forms, you should contact Ms Suzi Ericksen of the <br />Division's Denver office (see letterhead) to help you eet started. <br />Exhibit S -Permanent Manmade Structures <br />Please submit all damage agreements for structures owned by outside parties as soon as possible and in [he format <br />described in the Rules. Please note that they must all be bilaterally signed, dated and notarized. <br />Please provide information about the telephone line, and ensure that you obtain a damage aereemetrt for the phone <br />line too. <br />In lieu of obtaining a damage agreement, if you plan to operate in any way within 200 feet of a structure, you may <br />elect to submit an engineering anaysis showing no adverse impact to that structure. <br />Other Items <br />The decision date is still set for December l2, 2000. I must remind you that if at the end of business on the <br />decision date }•our application is not adequate for approval, it may be denied. If additional time is needed to <br />prepare the required response materials, an extension may be requested. <br />Until the application is approved, please be reminded that this letter and all subsequent responses from you must be <br />additionally filed with the county clerk and recorder, and the receipt for such filing fonvarded to me prior to the <br />decision date.
