May 18, 2004

NATURAL SODA, INC.
3200 County Rd. 31
Rifle, Colorado 81650
1/
Mr. Gregg Squire
Division of Minerals and Geology
Department of Natural Resources
1313 Sherman Street, Room 215
Denver, CO 80203

Phone: (970) 878-3674
Fax: (970) 878-5866
RECEIVED
MAY ; 9 2004
Division of Minerals and Geology
Subject: Notification for Plugging and Abandonment of Injection Wells
4-3H, 2B-3C, 3A-4H, lA-4HI and Conversion of Production Well 2A-
2V to aRaw Water Supply Well
Dear Mr. Squire:
Enclosed please find BLM Sundry Notices and well diagrams for injection wells
4-3H, 2B-3C, 3A-4H, lA-4HI, and production well 2A-2V. Also enclosed, please
find EPA Plugging and Abandonment or rework forms for the above wells. The
approved DMGBLM Mine Plan and EPA iTIC Permit require the plugging and
abandonment or conversion.
The plugging and abandonment of the four injection wells is simplified by the fact
there is cement to surface on the backside of the casing. The completion involves
the removal of tubing sufficient to assure placement of at least 200 feet of cement
between the mining zone and the B-Groove aquifer. A cast iron bridge plug will be
set, followed by a cement plug. The first stage of cement will be tagged and then
the remaining casing will be filled with cement to surface. The plugging and
abandonment plans attached meet or exceed the requirements of the Mine Plan and
EPA UIC Permit.
