DMRQA 23 Instructions for Permittees <br />1. If you use a contract lab far your NPDES work, please confirm they have received their PT <br />standards from Wibby Environmental. <br />2. Have your in-house laboratory andlor contract analyze the DMRQA or WP samples received by <br />Wibby Environmental and fill in the Data Reporting Sheets enclosed with the PT samples. Please <br />note, there are three Wibby Environmental PT studies that may be used for the DMRQA 23: <br />DMRQA 23 -opens June 2, 2003, closes August 29, 2003. <br />June WP (Study WP0603) -opens June 9, 2003, closes July 24, 2003. <br />Jufy WP (Study WP0703) -opens July 14, 2003, closes August 28, 2003. <br />3. Have your in-house laboratory and/or contract laboratory send a copy of the Wibby <br />Environmental Data Reporting Sheets to you. Consolidate all of your results onto one set of Data <br />Reporting Sheets. If you need blank copies of the Wibby Environmental Data Reporting Sheets, <br />you can download them at www.wibbv.com. <br />4. Complete the NPDES Permittee Data Report Form (the four-page form supplied to you by the <br />EPA in your DMRQA 23 study announcement package). If you need another copy of the Form, <br />you can download them from www.wibbv.com. <br />5. Attach the NPDES Permittee Data Report Form to the Wibby Environmental Data Reporting <br />Sheets. This is vour completed "DMRQA 23 Data Packaoe.° <br />6. Mail a copy of your "DMRQA 23 Data Package" to your state and Regional DMRQA coordinator. <br />Their name and address is included in the EPA package sent to you. <br />7. Mail the signed original "DMRQA 23 Data Package° to Wibby Environmental at 6390 Joyce Drive, <br />#100, Golden, CO 80403. You may also fax your Package to Wibby Environmental at 303-940- <br />0043. Maintain a copy for your records. Make certain you complete this step by the closing date <br />for the app/icah/e studv. The closing dates for the Studies are: <br />DMRQA 23 -August 29, 2003 <br />June WP (Study WP0603) -July 24, 2003 <br />July WP (Study WP0703) -August 28, 2003 <br />8. By October 31, 2003, Wibby Environmental will send you your final DMRQA 23 Study Report. A <br />copy will also be sent to your state and Regional NPDES Permit Authorities. <br />9. If any results are reported as °Not Acceptable," in the final report negotiate any corrective actions <br />with your state or Regional NPDES Permit Regulatory Authority and submit a signed corrective <br />action by December 8, 2003. <br />If you have any questions or need additional information, contact Wibby Environmental at <br />303-940-0033.
