If you're not sure that the mailing will reach them in time, you are welcome to fax the form to <br />RTC and then mail the hard copy the same day. RTC must receive a hard copy of your <br />NPDES Permittee Data Report Form for you to receive a Performance Evaluation (PE) <br />report. <br />The second enclosure is the data reporting form that we sent to RTC to report the results of <br />the quality assurance tests conducted in the SeaCrest laboratory. You do not need to <br />send this form to RTC. It has already been sent by us (the testing laboratory) and <br />provides them with al{ the information they need for the performance evaluation. This form <br />has been enclosed just for your information. <br />The third piece of information is a page describing the "Permittee Reporting Responsibility". <br />I have enclosed this in case it was not something you received with your information <br />packets. It provides a lot of information about the testing, what your responsibilities are, <br />including deadlines, and when you can expect to receive results of the evaluation. <br />Once again, the provider, chosen by The SeaCrest Group, for the toxicants for DMRQA <br />Study 23 was Resource Technology Corporation in Laramie, Wyoming.
