Box 591 <br />Somerset, Colorado 81434 <br />Dear Henry: <br />RECEIVED <br />NOV 03 2003 <br />Division of Minerals 8 Geology <br />Enclosed you will find the information that you must submit for the 2003 Water Pollution <br />Laboratory Performance Evaluation Study WET 23 (DMRQA 23). There are three pieces of <br />information provided. The first and most important is the "NPDES Permittee Data Report <br />Form". The Permittee Data Report Form you have been sent is already filled out with our <br />RTC Lab Code and EPA Lab Code information, along with our mailing address, and the <br />analyte codes marked for the study tests that are required by your permit(s). The analyte <br />codes stand for the acute and/or chronic test information provided. <br />You will need to fill in the information requested at the top of the "NPDES Permittee Data <br />Report Form": the Permittee name; the mailing address where the study results should be <br />sent; the name of the contact person, their title and phone number; and the NPDES permit <br />number and extension number, if applicable. One form should be filled out for each <br />discharge permit. Those with multiple permifs have been sent forms for each permit. <br />However if an error has been made and you have not received enough forms, you can <br />make a photocopy of a form that has the correct analyte codes for your permit and use it. <br />You can check these on the RTC data report form (the second form enclosed) under the <br />column "Analyte No." Once the header information has been filled in, the NPDES Permittee <br />Data Report Form should be mailed to Resource Technology Corporation (RTC) at the <br />address given on the form and on the "Permittee Reporting Responsibility" sheet (third item <br />enclosed): <br />DMRQA Testing Coordinator <br />Resource Technology Corporation <br />P.O. Box 1346 <br />2931 Soldier Springs Road <br />Laramie, Wyoming 82070 <br />or the form can be faxed to "DMRQA 23 Testing Coordinator, R.T. Corporation" at <br />(307) 745-7936.
