e. Where semi•emual site inspections are shown in the plan to be impractical for inactive sites (sites where
industrial activity is no longer conducted), site inspections required by this apart shall be conducted at
appropriate intervals specified in the plan, but, is no case lase than once in three years. At lease one site
inspxtion nxluired under this paR shall be conducted prior to October 1, 1994 or the date two years after such
site becomes inactive, whichever is earlier.
4. Additional SWMp' R~quiremmts
a. Signatory F:equir®ents: A copy of the plan shall be signed is accordance with Part I.D. t0.a, and retained
on site. A certification that the plan has been completed and implemented shall be signed end submitted to the
Division in .accordance with SWMP Deadlines, PaR I.C.6.a, below.
b. SWMP Renew/Changes: Upon review of the SWMP, the Division may notify the permittee at any time that
the plan does not meet one or more of the minimum requirements of this permit. After such notification, the
permittee shall make changes to the plan and shall submit to the Division ett update to the plan including the
requested changes. Unless otherwise provided by the Division, the permittee shall have 30 days after such
notification to both make the necessary changes to the plan sad to implement them.
The permittx shall amend the plan whenever there is a change in design, construction, operation, or
maintenance which has a significant effect on the potential for the discharge of pollutants to the waters of the
State, or if iLe SWMP proves to be ineffective in achieving the general objectives of controlling pollutants in
stormwater discharges associated with industrial activity. Ameadmenls to the plan may be reviewed by the

It is the permittee's responsibility to notify the Colorado Division of Mining and Geology (formerly the Mined LanJ
Reclamation Division) of asy aigaificant changes at their site resulting from the implementation of the SWMP.
5. SWMP Availability
A copy of the SWIdP shall be provided to the Division, the Colorado Division of Mining and Geology (CDMG), andlor 4~
EPA upon request, and within the time frame specified in the request.
All SWMPs required under this permit are considered reports that shall be available to the public under Section
30S(b) of the CWA. Tae owner or operator of a facility with atormwater discharges covered by this permit shall
make plans available to members of the public upon requrst. However, the permittx may claim any portion of e
stormwafer polluti~m plea as confidential in accordance with 40 CFR Part 2.
6. SWMP Deedlints. Cettif~retion
a. Certifiratian of SWMP Completion and Lnplenentatioo: For facilities arisfirtg on, October 1, 1992, as
soon as possible but no later than the amiversary date of their Mined Lend RecLmation permit that falls
between July 1, 1993 and June 30, 1994, the permittee shall have implemented and be in complimce with
their SWMY.
Certification of compliance with the SWMP shall be submitted to the Division on or before the anniversary
date of the Mined Land Reclamation permit that falls betwxn July 1, 1993 and June 30, 1994.
For any existing facilities covered under this permit which do not also have a reclamation permit under
CDMG, the. SWMP certification of completion and implementation is due to the Division by July 1, 1993.
For new facilities (beginning operations after October 1, 1992), and for all other facilities applying after
July 1, 1993, the SWMP shall be prepared prior W applying for coverage coder the general permit, sad
